General possible clutch failure?

Hi guys - any help would be appreciated here

Have an mk2 55reg 72km on the clock - car seems to judder harshly when pulling off & when in high speeds the rev counter goes up more whilst the mph stays the same. It's been a pretty well looked after car tbh and I can't really think of anything other than a potential clutch change

sounds like clutch needs replacing. As flisko says it's slipping
