I Have heard that Fiat gearboxes are a little weak, I recently had a clutch fitted in my 1.2 Doblo, since then there has been a slight rattle from the box at tick over, this goes if you dip the clutch or raise the revs from just above tick over at 1000rpm it's gone.
It would make me think the box has been allowed to hang in the input shaft, there is also a very occasional drip of gear oil from the bell housing and a slight smell of gear oil.
If the box is going down the tubes, I will sell the car as it seems to need attention all the time with always something going, fortunately it has been a lot of silly low cost items recently, but I couldn't afford a gearbox.
What is your thoughts on the box, is this the way they fail?
