A hole is drilled to let a small amount of gas through, ie to fool the ecu to prevent a warning light coming on.
Newer cars tend to need them.
Most multiplas tend to not need the hole, as they have ancient systems...
 
I was more under the impression that it was to improve driveability and prevent hesitation around 2000rpm by creating turbulence in the gas flow. Hence the name "Swirl Plate".

Theres two different methods, one hole is for fooling the ecu, ie a single hole just big enough, and a swirl design, ie a number of holes to create a swirl effect as Davren advised.
Practically, I havent felt the need for a swirl plate on a 110jtd or 115 elx, a full blanking plate seems to have done the job.
