Part of the problem the NHS faces in claiming back fees from foreign visitors is that it just isn't set up to take in money. There was a story recently in the press that an American tourist was injured in an accident over here and on discharge couldn't find anybody to take the money he was willingly offering. I think it was a case that staff couldn't tell him how to pay and were unable to give him a contact number for Accounts so he could ring up later.

In other words, the mechanism just wasn't in place for him to make payment. There is a well run and effective department for paying invoices but none for making them up
 
That is precisely the point. It is though asking for money is grubby and not something the NHS should get involved with. I do not accept that, as somebody has tried to argue, that it is not worth the trouble.
