Hi, I've done this mod to my seicento and believe it makes a difference. I'm running a 1242cc Spi engine with Induction kit, 38mm throttle, some head work, 866 cam, McKritch etc. (All on tight budget!). I had a friend cut the flange off and re-weld it as close to the flexy coupling as possible. It needs to be at angled about 20 degrees to mate nicely with the cat pipe.
If you're thinking of fitting this to an 1108 engine, the flange could be about 10mm lower than mine (difference in height of engine block). That would mean a greater offset to account for between the coupling flange and the cat pipe.
Of course, you may have other ideas for the cat pipe, but I wanted to keep my set-up standard from the cat, backwards.
Another thing to watch for is keeping enough clearance over the anti-roll bar and avoiding pushing the exhaust too far off centre, so it bangs on the body. I had to spin the clamp at the back of the cat to get enough clearance.
