Do you mean what caused the overheating or has it overheated again? Cause is generally people not changing coolant/ corrosion inhibitors at service intervals and/ or a fault such as the radiator fan not switching on causing the engine to overheat and warp the head. If it has overheated again after changing the head gasket, did you have the head skimmed?
I guess you just have to take it apart and check the head gasket again, see if you can see where it's blown. Perhaps the head is cracked. Or perhaps the gasket was faulty (or upside down!)
Also bear in mind that the coolant runs through the inlet manifold - the inlet manifold gasket seals off the coolant - so a leak there can fill the cylinders.

i will check about the gasket being upside down. a week after the engine overheated i went and started it up, and it started first time. after about 10 seconds it started running rough and water was coming out of the back end of the exhaust. i removed the engine from the car took off the cylinder head and cylinders were flooded with water. i got a new gasket and ran water through the engine via a hose and the water just went straight to the cylinders again. i checked the block very closely and each cylinder, there seems to be no cracks anywhere...... How can i test if the inlet manifold is faulty?
You haven't said whether you skimmed the head or not. When the engine originally overheated the head may have become slightly warped. If it had and you didn't get it skimmed before fitting a new head gasket it will overheat and blow the new gasket.
no the head was not skimmed yet. But i am trying to eliminate all the other possibilities before skimming it even tough it has to be skimmed before reassembling the engine. Can u think of anymore problems that can be doing this to the cylinders. Do you know how i can test the intake manifold??
