I own a 2009 Ducato camper. It's done 86,000 miles, mainly on the continent. I suspect the top bearing on the front suspension is knocking. My previous van (old shape) had these fail and were replaced twice in its life. Are the newer vans just as prone to this problem? Is replacing them as bad a job as the old type were?

The Fiat engineers were quite satisfied with it, so yes, it is the same construction.
Before you start dismantling, check the wheel bearing. It is a single bearing now, non adjustable and prone to wear. Fortunately a large hydraulic press and the propel dowel is needed to replace it, so you probably cannot do that yourself.
 
My 2012 Euro 5 had new ones fitted at about 1 yr old, knocked on steering operation, was told they had been modified again since mine was built, since then there behaving, 3 yrs old in a month, 294000 miles, and with the roads as bad as they are pothole wise they've held up well,
Wishbones showing wear (cracking) will be replaced before mot that all it's had/needed on suspension
