Technical 09 Ducato 3.0 JTD rough starting

When starting the engine will fire then immediately stop very roughly unless I keep on turning it over with the starter and applying a little pressure on the throttle pedal. Could it be faulty throttle body? Earth strap renewed! Once running the engine is smooth.

No chance of a slight air leak at the fuel filter, or heater plug fault/slight white smoke from cold?
Any error codes read?
I know these are more common on older diesels but can still have relevance.
