It seems most vans have rear drum brakes. If I wanted to replace the rear discs on mine, how would I remove them once the calipers are swung out of the way?
Do they come away when the two locating studs for the wheels are removed, or is there something else I'm unaware of?

You have to let off the handbrake to take them off, so you'd want wheels properly chocked.

Other possible complications:
If they haven't been fitted with anti seize / copperslip they may be rusted to the hub which usually requires a large persuader.

If the disc doesn't turn freely when you let off the handbrake you may have to back off the adjuster using a screwdriver through the holes for the wheel studs.

While its off clean and grease the caliper sliders and brake pads and shoe contact points.

So, do you have to take the wheel locating studs off before you apply the hammer?

I'm thinking your question, which confused me a bit and so might confuse others is, have you got discs and callipers on your rears, or hubs and shoes?

Hubs and shoes, the handbrake cable operates the braking shoes (after adjustment).

Discs and callipers, the handbrake operates their own shoes inside the hub.
 
Just to expand on Shingy's post, the discs are integral with the 'top hat' casting that covers the handbrake shoes so the whole lot will come off as one.

I believe the wheel locating studs retain the disc to the hub so have to be removed.

Do you use the silicon brake grease for the sliders? I have tried to get some but finding it hard to get in the factors!

I'm afraid it's as Crossingkeeper said, the whole lot has got to come off, as the shoes for the handbrake are hiding under the "top-hat". Are you sure the handbrake shoes need replacing as they only hold the wheels from turning whilst stationary, so they aren't really doing too much "work" as such. Perhaps that whole area needs freeing up due perhaps to inactivity? Try disconnecting the handrake cable from the assembly and see if you can get it operating by hand. WD40 comes to mind.
