I'd check the rear brake shoes as they might be worn right out; if they're OK then your handbrake cables are stretched and need replacing.

The automatic adjusters in rear brake need clicking up - slacken off handbrake cables completely and pump brake pedal with engine running, you should hear clicking sound from rear wheels each time you pump brake pedal- continue until no more clicks then adjust handbrake cables until lever comes up 3 or 4 clicks. I replaced cables and rear shoes on my Punto before I found out that this was real problem all along. Handbrake was crap before, is pretty good now.
