Have you got the recirculate set for fresh air? Or recirculate? If no fresh air, you will gently suffocate as you use up the oxygen.

Next, check the air intake system. Under the windscreen, outside, is a grille to allow air into the heating system. Below that is a space that will get filled up with dead leaves. If the leaves, or other airborne debris block the heater intake, you get little or no fresh air.

Does it have aircon? If so, when was the pollen filter last changed. If blocked, again no fresh air.

Does the tailgate seal actually seal properly? If not, car will draw exhaust fumes into the cabin due to the low pressure at the back of the car. Talc on the seal, gently push the tailgate against it, then lift and check it has touched all around.

warm only, does it have a musty acrid smell, could be a slight leak in the matrix? are you losing coolant?

the cabin heater is a small radiator, also called a matrix. On left hand drive cars it's easily accessible from the glovebox if you want to check for dampness.

While this is most likely a coolant leak it could be carbon monoxide (CO)poisoning from exhaust fumes which can be fatal. You need to make sure it's not this. If you or your wife look pink or flushed when in the car, this is a sign of serious CO poisoning, get out into fresh air and seek medical attention. You can get CO detectors either one-time chemical or electronic. The chemical ones are typically a card wit a spot that darkens. Not so common these days but can be found in Camping / Caravaning stores or boating suppliers. Electronic ones are similar to smoke detectors and are sold in hardware / DIY stores and some supermarkets.
If it is CO it could be coming from a leaking exhaust, bad rear door seals leaking heater duct or a combination of these.

A "gas mask" (filtered respirator) will not work for CO. There is no effective sorbent. One might remove the smell, but it won't remove CO (which has no smell) For protection you would need a mask supplied from a clean air supply. Antifreeze fumes aren't good for you either but don't have the low level acute toxicity of CO.

I guess don't make the air recycled in the cabinet, I heard sometimes the air channels need to be cleaned by some spray products to get ride of the uncomfortable smells .
