They are the details/numbers you need to send funds abroad, the bank I work for print them on your statement.
 
swift and bic are the same thing, its your banks international sorting code- your bank branch provides this.

IBAN is your international bank account number. Usually made up of you sortcode combined with your account number

swift address is the address for the swift code, ie sub branches or smaller branches may use the same code as a larger branch, so its that branch address
 
BIC & IBAN are on the top right hand side of the statement, below your Roll Number, Sort Code & Account number.

The Swift address will be Payment & International Services address in Halifax but I can't remember it off the top of my head, you can call Halifax Direct on 08457 20 30 40 and they will be able to tell you. (and yes I work for the Halifax incase you were wondering)
