Doblo -oil in coolant exp tank

My 04 1.9 JTD doblo has thick oil sludge in the coolant exp tank.
The engine has no emusified oil/water on dipstick or in oil filler cap, and the oil filler hole only has dirty but pure oil in it.
I removed the oil cooler and got a good slug of pure clean coolant, and also a slug of good not contaminated oil. I have blown air through the coolant side of cooler and cant see any leaks.
Does any one have any suggestions.

Who tops your car fluids up? Had a recent service?

I've known garages give this meanial task to the tea boy who gets is wrong, although brake/power steering/oil into the coolant header tank is quite rare. Automatic transmission fluid into the washer bottle is more common.
